2015/2016 Simi Valley PTA/PTSA Council Honorary Service Award

Each year the Simi Valley PTA/PTSA Council has the privilege of giving special recognition to individuals for their outstanding service to children and our community.  A contribution in his or her name is made to the HSA Scholarship Fund for scholarships to assist graduating high school students, teachers and parent volunteers who are continuing their education.  These awards are presented at our annual Honorary Service Award dinner.  This year the event is scheduled for Friday,  May 13, 2016 at the Grand Vista Hotel. If you would like to recognize an individual, click the link below to print and complete the form. You may return to the form to the school office.
